(You qualify for the Young Adult Plan, MyHNE – but remember, this isn’t your only option.)
27-45 years old
45-65 years old

Why are you buying insurance?  Pick the answer that best describes you.
I have to buy insurance, so I want to pay as little as possible. I probably won’t need it anyway.
Insurance is less expensive and easier to get now.  I’m pretty healthy.  I don’t see doctors very often. If I do need to see a doctor, I want my insurance to help cover my medical bills.
I have a chronic condition.  I have good years and bad years.  Sometimes, I go a long time without seeing my doctor.  Others, it seems like I’m in there once a week.

Do you have a doctor whom you see regularly?  AND/OR Do you take any prescriptions?
No, I don’t currently see a particular doctor and I am not on any prescriptions.
I see the same doctor, but I’ll switch if I can get on a less expensive plan.
AND/OR
The only prescriptions I get are generic maintenance ones.
I have very specific medical and prescription needs.

How often do you see your doctor?
Very rarely
A few times a year
(You might want to select this answer if you have a chronic condition that it under control.  IN other words, maybe you don’t see your doctor that often now, but that could change if you chronic condition gets worse.)
Often

For any plan, you will have to pay a monthly premium.  There will also be out of pocket costs when you get covered services – like a deductible and/or copayment.  Choose the statement below that best describes you
I want the lowest premium rate available. I almost never see my doctor, so out-of-pocket costs are less important to me.
It’s okay with me to pay a higher premium if I pay less when I see my doctor. I’m a careful consumer and will balance premium costs with out-of-pocket expense.
I am willing to pay a higher premium if it means paying less to see my doctor as often as I need to. Low out of pocket expenses for services are most important to me.
